Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

Ricardo Serran Lobo is a Brazilian blogger who writes about his famous neighbor, the politician Roberto Jefferson, head of the Brazilian Labor Party in the Brazilian Congress of Deputies. Jefferson has become a major figure in the ongoing Brazilian mensalão scandal revolving around corruption and bribery.

Vizinho do Jefferson [1] quickly became very popular among Brazilians, describing the routine of Jefferson, while providing information about politics and fresh news about the scandal. Lobo’s blog got third place in the Best Of Blogs contest run by Deutsche Welle International.

Lobo gets an intimate look at the center of Brazilian politics by living in Brasilia, near residences of parliamentarians (including Roberto Jefferson), public buildings and the Brazilian Chamber of Deputies. Or, as he says in his blog: “near the eye of the hurricane,” a reference to the recent political crisis.

At first, Lobo began describing the daily activities of his famous neighbor deputy Roberto Jefferson. As the crisis moved on, he began to describe the political events regarding it. His writings evolved to not only what is going on with Jefferson, but what is going on in Brazilian Congress, and has interviewed politicians, including Roberto Jefferson himself.

The blog tries to be informative, with a lot of humour (common with Brazilians), and some protests against the bad habits of Brazilian politics in general.

Jefferson’s neighbor, the blog, is an example of citizen journalism and it shows that ordinary people can compete with professional media.

Diamonds: Anglo American to pay US$5.1 billion for 40% of De Beers

Anglo American PLC has negotiated a price of US$5.1 billion to buy 40% of diamond giant De Beers from the Oppenheimer family. Anglo’s stake will jump from 45% to either 75% or 85%; the Botswanan government has an option to increase its own stake.

“This has been a momentous and difficult decision as my family has been in the diamond industry for more than 100 years and part of De Beers for over 80 years,” said De Beers chairman Nicky Oppenheimer. The Oppenheimers are selling their entire remaining stake in the company they took over in the 1920s. They retain 2% of Anglo, founded in 1917 by Sir Ernest Oppenheimer. Until Nicky’s resignation this year Anglo’s board has always featured an Oppenheimer.

As-of June De Beers assets totalled US$8.2 billion with US$1.2 billion pre-tax profit over the prior six months, compared to a US$863 million pre-tax profit last year. The firm owns two Canadian mines and one in South Africa, along with joint ownership with Botswana in two of the world’s biggest diamond mines.

Part of the sale deal promises the Oppenheimers will be given part of any value increase if the firm is floated within two years. Nonetheless, Anglo CEO Cynthia Carroll denied this means a flotation is planned. She also said there is no connection between the purchase, which is set to be finalised next year, and Chilean state-owned copper firm Coldeco’s recent major purchase of 49% of various Anglo property. The Oppenheimers receive 20% of any price increase if a float occurs within a year, and 10% if one happens the year after.

Fur fans flock to Toronto’s Furnal Equinox 2019

From March 15 to 17, the Canadian city of Toronto played host to the tenth Furnal Equinox, an annual event dedicated to the “furry fandom.” Wikinews attended. Programming ranged from music to gender, science to art, covering dozens of aspects of the varied subculture. The event’s featured guests were visual artists Moth Monarch and Cat-Monk Shiro, as well as the co-owners of US fursuit costume builders Don’t Hug Cacti.

The event raised nearly CDN$11,000 for Pet Patrol, a non-profit rescue organization in Kitchener-Waterloo, Ontario, run by volunteers. This exceeded their goal of $10,000, the funds needed to finish a rural sanctuary. The furry community is well-known for their charitable efforts. Along with direct donations, the funds were raised through a charity auction offering original artwork, and a fursuit design by guests of honour “Don’t Hug Cacti.” Last year, Furnal Equinox raised funds for a farm animal sanctuary.

While only 10–15% of people within the fandom own a fursuit according to a 2011 study, event organizers reported this year 908 of the 2240 attendees at Furnal Equinox brought at least one elaborate outfit to the event. The outfits are usually based on original characters, known as “fursonas”.

Guests of Honour Cherie and Sean O’Donnell, known within the community as “Lucky and Skuff Coyote”, held a session on fursuit construction on Saturday afternoon. The married couple are among the most prominent builders in the fandom, under the name Don’t Hug Cacti. The scale of their business was evident, as Sean had made over a thousand pairs of “handpaws”, costume gloves.

The couple encouraged attendees to continue developing their technique, sharing that all professional fursuit makers had developed different techniques. They felt that they learned more from failed projects than successful ones, citing the Chuck Jones quote that “every artist has thousands of bad drawings,” and that you have to work through them to achieve. Cherie, known as Lucky, recalled receiving a Sylvester the Cat plush toy from a Six Flags theme park at age 10. She promptly hollowed the toy out, turning it into a costume. Creating a costume isn’t without its hazards: the company uses 450°F (232°C) glue guns. They’re “like sticking your hand in an oven.”

Other programming included improv comedy, dances, life drawing of fursuiters, a review of scientific research by a research group at four universities called FurScience, a pin collector’s social, and workshops in writing.

The “Dealer’s Den” hall was expanded this year, with even more retailers and artists. While many offered “furry” versions of traditional products, at least one business focused on “pushing the boundaries of fursuit technology.” Along with 3D printing a bone-shaped name tag when Wikinews visited, Grivik was demonstrating miniature computer screens that could be used as “eyes” for a fursuit. The electronic displays projected an animation of eyes looking around, blinking occasionally. The maker has also developed “a way to install a camera inside suit heads, to improve fursuiter visibility.” He hopes the tech would reduce suiting risks and accidents. Without the need for eyeholes, fursuit makers would have “more options for building different eyestyles.”

New York governor Spitzer tied to prostitution ring

According to a report by The New York Times (The Times), New York Governor Eliot Spitzer informed his senior administration officials on Monday that he was being investigated for using a “high-class” prostitution ring. The Times report cites anonymous officials who work for Spitzer. A law enforcement official who spoke to Associated Press on condition of anonymity stated that a federal wiretap related to a recent investigation revealed that Spitzer paid for prostitutes from an online service under investigation.

ABC News reported that, according to federal officials, the federal investigation into the prostitution ring was initially motivated by money transfers which led agents to believe Governor Spitzer was hiding bribes. Sources familiar with the case spoke to The Washington Post and CNN, and said that Spitzer is identified as “Client 9” in court documents and on a federal wiretap.

In a brief press conference held Monday afternoon, Spitzer apologized to his family and to the public, but did not directly admit or deny involvement in the prostitution ring. “I apologize first and most importantly to my family. I apologize to the public, to whom I promised better. I must now dedicate some time to rededicate my trust to my family,” he said, adding that he “acted in a way that violates his obligation to his family.”

Spitzer did not answer questions from reporters, indicating he would have more to say “in short order.” He did not admit or deny any allegations.

The Times’ report alleges that Gov. Spitzer was a client of the Emperors Club VIP, a high-priced international prostitution ring. Last week, federal investigators arrested four people involved with the operation in and around New York. The investigation utilized undercover work and wiretap recordings at various club locations, including New York and Washington, with the latter allegedly containing Spitzer’s voice.

Reports indicate the governor learned on Friday, March 7, of federal investigators’ discovery of his alleged involvement in the prostitution ring. He informed his top aides on Sunday night before meeting with the rest of his staff Monday, according to The Times. Sources for the Fox News Channel and WCBS-TV in New York report Spitzer could resign and be indicted, in which case the Lieutenant Governor of New York, David Paterson, would become governor.

James Tedisco, the New York State Assembly Republican Minority Leader, says Spitzer should resign and state Republican leadership will initiate impeachment proceedings if the governor fails to step down. “Today’s news that Eliot Spitzer was likely involved with a prostitution ring and his refusal to deny it leads to one inescapable conclusion: he has disgraced his office and the entire state of New York. He should resign his office immediately,” stated Tedisco.

The European Parliament has rejected the directive on the patentability of computer-implemented inventions (software patent directive) sustained by lobbies of large software publicists such as the corporations Microsoft, Siemens, Nokia and Alcatel, grouped under the title of the European Information & Communications Technology Industry Association (EICTA, [1]). The directive involved the granting of software patents.

648 MEPs out of 680 rejected the text, 18 voted for and 14 abstained.

A rejection vote became the expected outcome when the European People’s Party, initially in favour of the directive, decided to reject it.

The European Greens, Socialist Group and European Liberal Democrat and Reform Party also voted for rejection of the directive for heterogeneous reason. Michel Rocard, author of a number of amendments to the original directive, said that the majority of the modifications were unlikely to be supported by the Commission and Council, with whom the Parliament would have had to enter a Conciliation procedure if it had voted for maintaining the directive in moditifed form. “Better have no text at all than a bad one”, he added.

Before the vote, Rocard pointed at the irritation of the Parliament towards the Commission: “There is collective anger throughout the Parliament because of the way the directive was handled by the Commission and the Council”.

During the debate on Tuesday, Commissioner Joaquín Almunia told MEPs: “Should you decide to reject the common position, the Commission will not submit a new proposal.”.

The rejection was welcomed by small and medium software companies, as well as by Free Software supporters. The Directive had been subject to an intense campaigning, within the Parliament, in the news media and on the Internet. The supporters of the Council position appear to have spent several ten millions, hiring prestigious PR agencies with at least 30-40 lobbyists who roamed the halls of the Parliament every day for 3 months, and many full-page advertisements in EU newspapers such as European Voice, EU Reporter etc. The opponents of software patentability (that is supporters of the position taken by the European Parliament in its 1st reading of 24 September 2003), coordinated under the roof of the Foundation for a Free Information Infrastructure (FFII), also had several lobbyists stationed in Brussels, conducted several conferences and demonstrations and published some newspaper advertisements, with a total budget of nearly 100,000 eur apart from countless unpaid working hours of a dedicated supporter base, consisting mainly of programmers and software entrepreneurs.

Times Square bomb suspects arrested in Pakistan

At least six suspects, wanted in connection with a failed car bomb attempt in Times Square, New York City on May 1, have been arrested by authorities in Pakistan. The arrests follow a visit to Pakistan by two high-ranking American security officials—US National Security Adviser Gen James Jones and Leon Panetta, director of the Central Intelligence Agency (CIA), both of whom praised Pakistan for its efforts in tracking down suspects in the bomb attack.

Amongst those arrested are the co-owner of a large company which provides catering to several embassies in Pakistan and a major in the Pakistani Army, though the Army released a statement denying any involvment. Some are believed to have been educated in the United States. A notice on the website of the US embassy in Islamabad reported that the catering company co-owned by one of the suspects had links to terrorism and has since advised US citizens against using the firm.

Those arrested are believed to have had links with Faisal Shahzad, the primary suspect in the bombing, who was arrested on May 3 on board an aircraft at New York’s JFK Airport.

One source told Reuters: “We are investigating whether Ashraf has provided any financial support to Faisal because Ashraf and his father are rich people and they run a very big catering business” though another stated that “[t]hey may be innocent because being friends does not mean you are involved in the activities of your friends”.

The suspects are currently being detained by the Pakistani security services, who are known to have close links with the CIA, to the extent of allowing CIA officials access to prisoners.
